FORM-A-Tool INC
Founded in 1947, Form-A-Tool Co., Inc. began when James E. Musacchia rented a modest two-car garage behind the Madison Theater on Cleveland’s west side near West 95th Street, launching what started as a small grinding service.
From those humble beginnings, the company grew steadily- expanding from 500 square feet to more than 45,000 square feet, from two employees to 150, and from just two grinding machines to more than 350 machines across 24 specialized departments.
In 1979, Form-A-Tool entered a new phase of growth under the leadership of James E. Musacchia Jr. (“Jimmy”), with the construction of a state-of-the-art manufacturing facility in Piedmont, South Carolina. Over the next several decades, Jimmy expanded the company’s footprint throughout the Southern United States, supplying high-quality tooling solutions to a wide range of clients.
Following Jimmy’s passing in 2023, leadership transitioned to his wife, Kate Musacchia, who continued the company’s commitment to quality, innovation, and customer service.
A new chapter begins in 2026, as David Lasalla- grandson of founder James E. Musacchia- assumes leadership, carrying forward the legacy established by his grandfather and uncle.
Today, Form-A-Tool proudly continues a tradition of excellence spanning more than seven decades of trusted service and precision craftsmanship.
